A chaotic scene unfolded Friday morning on the border of River Forest and Oak Park, leaving one man dead and 14 others injured following a high-speed collision and a subsequent self-inflicted gunshot wound.

The incident began at approximately 8:56 a.m. on April 3, when 48-year-old Thomas E. Cremeens of Plainfield was traveling southbound on Harlem Avenue in a Toyota 4Runner. According to the River Forest Police Department, surveillance footage captured the vehicle approaching Lake Street at what investigators describe as a “high rate of speed.”

The Collision

Authorities believe the chain of events was triggered when a pedestrian attempted to cross Harlem Avenue eastbound from the southwest corner. In an apparent attempt to avoid the pedestrian, Cremeens swerved into the northbound lanes, sparking a multi-vehicle pileup.

The 4Runner collided with another passenger vehicle and a Pace bus, ultimately involving a total of four cars and the transit bus.

A Tragic Turn

In the immediate aftermath, witnesses rushed to the scene. A “good Samaritan” who approached the 4Runner to check on the driver’s well-being reported hearing a single gunshot after stepping away from the vehicle.

The first Oak Park police officer to arrive discovered Cremeens slumped over with a visible head wound; a firearm was recovered from the floorboard of the SUV. Cremeens was pronounced dead by a doctor from Loyola University Medical Center at 9:13 a.m.

Casualties and Road Closures

While the driver’s death appears to be a self-inflicted gunshot, the collision itself caused widespread injuries:

• 14 passengers in the surrounding vehicles sustained injuries.

• 11 individuals were transported to local hospitals, including Rush Oak Park, Loyola, MacNeal, and Gottlieb.

• 3 individuals were treated and released at the scene.

Officials confirmed that none of the injuries to the 14 passengers are currently considered life-threatening.

Harlem Avenue remained closed for several hours as the River Forest and Forest Park Police Departments assisted with traffic control and scene processing. The roadway was officially reopened to the public at approximately 2 p.m.
